This notice summarizes how we protect medical information and personal information, and how you may exercise your rights. A full notice is available upon request.

Your privacy matters. We protect your health information, California medical information, consumer information, and sensitive information according to HIPAA, California law, 42 CFR Part 2 when applicable, and our privacy and security policies.

  1. HOW WE MAY USE AND DISCLOSE INFORMATION

We may use and disclose information for treatment, payment, and health care operations, such as care coordination, billing, quality review, licensing, compliance, and patient support.

We may also use and disclose information when required or permitted by law, including public health, legal, regulatory, and safety-related purposes.

When written authorization is required, including certain sensitive disclosures, we will request your written authorization.

  1. SPECIAL CALIFORNIA PRIVACY PROTECTIONS

California law may provide additional protections for medical information, mental health, HIV/AIDS, genetic information, reproductive health, minors, and sensitive services.

When California law, HIPAA, 42 CFR Part 2, or another law is more protective, we follow the stricter requirement.

We review unusual requests, subpoenas, law enforcement requests, and sensitive-record disclosures before releasing information.

  1. SUBSTANCE USE DISORDER RECORDS

Certain substance use disorder diagnosis, treatment, and referral records may receive special protection under 42 CFR Part 2.

These records generally require written consent before disclosure, except when a specific legal exception applies.

Part 2 records may not be used in legal proceedings without specific written authorization or a qualifying court order.

  1. YOUR PRIVACY AND CONSUMER RIGHTS

You may ask to inspect or obtain a copy of your health record, request an amendment, request confidential communications, and request restrictions where available.

Where California consumer privacy laws apply, you may have rights to know/access, correct, delete, opt out of sale or sharing, limit certain sensitive personal information, and use an authorized agent.

You may request an accounting of certain disclosures and may file a complaint without retaliation.

  1. WEBSITES, APPS, COOKIES, CHATBOTS, AND AI

We review website, mobile app, cookie, pixel, chatbot, portal, advertising, and analytics technologies before use with patient or consumer information.

AI tools that access, create, summarize, infer from, store, or transmit health information must be approved, safeguarded, and monitored.

Do not place medical information, Part 2 records, or other sensitive information into public or unapproved AI tools.

  1. OUR RESPONSIBILITIES

We must maintain the privacy and security of protected information, provide notices of our practices, and follow the terms of our current notices.

We must notify affected individuals when a breach notification is required by law.

We train our workforce, use contracts with vendors when required, and maintain records of privacy and security decisions.
